<p><u><a href="http://www.medicinenet.com/script/main/art.asp?articlekey=9651">Precocious Puberty</a></u> -- When the signs of puberty (development of breasts, <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=8974">testes</a>, pubic and underarm hair, body odor, menstrual bleeding, and increased growth rate) appear before the age of 8 years in a girl or 9 1/2 years in a boy, it is termed "<a href="http://www.medicinenet.com/script/main/art.asp?articlekey=9647">precocious</a> puberty." In the most common form of precocious puberty, there is early activation of the regions in the brain which control the maturation of the gonads (ovaries in a girl and testes in a boy). One brain center, the <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=3866">hypothalamus</a>, secretes a substance called gonadotropin-releasing-hormone or "GnRH." This acts, in turn, on another part of the brain, the <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=4915">pituitary gland</a>, to cause increased secretion of hormones called "gonadotropins" (LH and FSH) that travel through the bloodstream, and act on the ovaries or testes to stimulate secretion of <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=3329">estrogen</a> or <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=5747">testosterone</a>.
<p>The precocious puberty in affected girls is caused by <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=15837">estrogens</a> which are secreted into the bloodstream by <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=8432">ovarian cysts</a>, which enlarge, and then decrease in size over periods of weeks to days. The cysts can be visualized and measured by ultrasonography, in which sound waves are used to outline the dimensions of the ovaries. The cysts may become quite big, occasionally over 50 cc in volume (about the size of a golf ball). Frequently, menstrual bleeding and breast enlargement accompany the growth of a <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=2888">cyst</a>. In fact, menstrual bleeding under 2 years of age has been the first <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=5610">symptom</a> of polyostotic fibrous dysplasia in 85% of patients. Although <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=11805">ovarian</a> cysts and irregular menstrual bleeding may continue into adolescence and adulthood, many adult women with the syndrome are <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=3411">fertile</a>, and can bear normal children.
<p>The precocious puberty in the syndrome has been difficult to treat. After surgical removal of the cyst or of the entire affected <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=4705">ovary</a>, cysts usually <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=5255">recur</a> in the remaining ovary. A progesterone-like <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=3783">hormone</a> called Provera can be given to suppress the menstrual bleeding, but does not appear to slow the rapid rates of growth and bone development, and may have unwanted effects on adrenal functioning. The synthetic forms of GnRH (Deslorelin, Histerelin, and Lupron) which suppress LH and FSH, and are used to treat the common, gonadotropin-dependent form of precocious puberty, are not effective in most girls with the syndrome.</p>
<u><a href="http://www.medicinenet.com/script/main/art.asp?articlekey=19509">Thyroid</a> Function</u> -- Almost 50% of patients with the syndrome have <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=5778">thyroid gland</a> abnormalities. These include generalized enlargement called <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=3615">goiter</a> and irregular masses called nodules and cysts. Some patients have subtle structural changes detected only by ultrasonography. Pituitary thyroid-stimulating-hormone (<a href="http://www.medicinenet.com/script/main/art.asp?articlekey=5858">TSH</a>) levels are low in these patients, and thyroid hormone levels may be normal or elevated. <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=10897">Therapy</a> with drugs which block thyroid hormone <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=15508">synthesis</a> (<a href="http://www.medicinenet.com/script/main/art.asp?articlekey=5070">Propylthiouracil</a> or Methimazole), can be given if thyroid hormone levels are excessively high.
<p><u><a href="http://www.medicinenet.com/script/main/art.asp?articlekey=16691">Growth Hormone</a></u> -- Excessive secretion of pituitary growth hormone is sometimes seen in the syndrome. This may be diagnosed in young adults, when they developed coarsening of facial features, enlargement of hands and feet, and <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=2337">arthritis</a> characteristic of the condition termed "<a href="http://www.medicinenet.com/script/main/art.asp?articlekey=2127">acromegaly</a>." Therapy has included surgical removal of the area of the pituitary which is secreting the hormone, and use of new, synthetic analogs of the hormone somatostatin, which suppress growth hormone secretion.
<p><u>Other Endocrine Abnormalities</u> -- Although rare, adrenal enlargement and excessive secretion of the adrenal <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=3786">hormone cortisol</a> is seen in the syndrome. This may cause obesity of the face and trunk, weight gain, skin fragility and cessation of growth in <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=16600">childhood</a>. These symptoms are called "Cushing's syndrome." Treatment is removal of the affected adrenal glands, or use of drugs which block <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=2850">cortisol</a> synthesis.
<p>Some children with polyostotic fibrous dysplasia have very low levels of <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=4880">phosphorus</a> in their blood due to excessive losses of <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=4879">phosphate</a> in their urine. This may cause bone changes associated with <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=6714">rickets</a>, and may be treated with <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=4655">oral</a> phosphates and supplemental <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=15296">vitamin D</a>.</p>
</li>
<li><u>Bone Disease -- Polyostotic Fibrous Dysplasia</u>

<p>The term "polyostotic fibrous dysplasia" means "abnormal fibrous <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=5800">tissue</a> growth in many bones." Normal bone is replaced by irregular masses of fibroblast cells. When this occurs in weight-bearing bones, such as the <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=3407">femur</a> (<a href="http://www.medicinenet.com/script/main/art.asp?articlekey=8456">upper leg</a> bone), limping, deformity, and fractures may occur. In many children, the arms and/or legs are of unequal length, even in the absence of actual <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=3513">fracture</a>. Regions of fibrous dysplasia are also very common in the bones that form the skull and upper <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=4071">jaw</a>. If these areas begin to expand, skull and facial asymmetry may result.

<p>Polyostotic fibrous dysplasia can often be seen in a plain <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=6032">X-ray</a> picture of the <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=5503">skeleton</a>. A more sensitive method of finding lesions is a <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=2506">bone scan</a>, in which a small amount of radioactivity (an <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=4059">isotope</a> of technetium) is injected into a <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=5970">vein</a>, taken up by the abnormal tissues, and detected by a scanner.

<p>Some children may be minimally affected, with no asymmetry, deformity or fracture, and lesions detected only by a bone scan. In a few children, lesions are found only in the base of the skull. By repeating bone scans at intervals of 1 to 2 years, it has been shown that the bone disease in some children may become more extensive over time. Unfortunately, severe bone disease can have permanent effects upon physical appearance and mobility.

<p>There is no known hormonal or medical treatment effective in controlling <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=10697">progressive</a> polyostotic fibrous dysplasia. Surgical procedures to correct fracture and deformity include grafting, pinning, and casting. Skull and jaw changes are often corrected surgically, with great improvement in appearance. Treatment and therapy for this bone disease is usually the most difficult aspect of caring for a child who has severe polyostotic fibrous dysplasia.</p>


<li><u>Skin Abnormalities</u>

<p>The irregular, flat areas of increased skin pigment in the syndrome are called "cafe-au-lait" spots because, in children with light complexions, they are the color of coffee with milk. In dark skinned individuals, these spots may be difficult to see. Most children have the pigment from birth, and it almost never becomes more extensive. The pattern of the pigment distribution is unique, often starting or ending abruptly at the midline on the <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=2081">abdomen</a> in front or at the <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=5529">spine</a> in back. Some children have no cafe-au-lait pigment at all; in a few, it is confined to small areas, such as the nape of the <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=16929">neck</a> or crease of the buttocks. There are seldom any medical problems associated with the areas of cafe-au-lait pigment. Some adolescent children may want to use makeup to obscure areas of dark pigment on the face.

<p>There is no way to accurately predict how severe the disease may become in an affected child. There are no reported cases of any parent being affected, and the children of women with the syndrome are normal. All races appear to be affected equally.

<p>The mystery of the cause of polyostotic fibrous dysplasia appears to have been solved by the identification of activating mutations in the GNAS1 gene. The activating mutations render the GNAS1 gene functionally constitutive, turning the gene irreversibly on, so it is constantly active. This occurs in a mosaic pattern, in some tissues and not others.</p>

<p>The syndrome was first described independently in the 1930s by the American pediatrician Donovan James McCune (1902-1976) and a team from the Massachusetts General Hospital in Boston led by the great endocrinologist Fuller Albright (1900-1969):

<p><b>Bronchopulmonary dysplasia:</b> A chronic lung disease of babies, which most commonly develops in the first 4 weeks after birth and most often affects babies born at least 4 weeks before term. The lungs do not work properly in bronchopulmonary dysplasia (BPD) and the baby has trouble breathing, needs extra oxygen, and may need help from a ventilator (breathing machine).
<p>BPD is different from respiratory distress syndrome (RDS), which occurs in premature infants because their lungs do not produce enough surfactant (a liquid material that coats the inside of the lungs making it easy to breathe). Babies with RDS are treated with surfactant replacement therapy and generally do well.
<p>The risk of BPD increases as the baby's weight and time in the mother's uterus decreases. Today, most babies with BPD (9 out of 10) weigh 1,500 grams (about 3 and a half pounds) or less at birth. Babies born weighing less than 1,000 grams (about 2 pounds) are at very high risk. About 1 out 3 of these gets BPD.
<p>BPD does not develop in all infants for the same reasons. Factors predisposing to BPD include immaturity of the lungs, exposure to a high level of oxygen, time on a ventilator, and infections such as <A href="http://www.medicinenet.com/script/main/art.asp?articlekey=450">pneumonia</a>. About 5,000 to 10,000 babies in the US get BPD each year. Because more babies weighing less than 3 pounds live past 4 weeks, more babies get BPD today than 30 years ago.</p>
<p>Babies with BPD struggle to breathe and show signs of respiratory distress including rapid, shallow breathing (tachypnea), sucked-in ribs and chest (retraction), cough. movement of the chest and stomach (abdomen) in the opposite direction with every breath (see-saw breathing), wheezing (a whistling sound when the baby breathes),
"craning" the neck (raising or stretching the neck by using the neck muscles to push more air into the lungs), and repeated periods of "bluing" (cyanosis) due to the low amount of oxygen in the blood.
<p>The treatment of RDS includes the use of: a ventilator until the baby can breathe on its own; extra oxygen until the baby can breathe normally on room air; bronchodilator drugs to improve the flow of air in and out of the lungs; corticosteroids to reduce the swelling and inflammation of the airways; keeping a close watch on the amount of fluids the baby is taking in to avoid fluid build-up in the lungs; diuretics to help remove extra fluid from the lungs; antibiotics to control infections; IV feedings to provide extra <A href="http://www.medicinenet.com/script/main/art.asp?articlekey=10192">nutrition</a> to help the baby recover; special feedings and formula; and physical therapy to help make the baby's chest muscles grow stronger and to keep the lungs clear of mucus.
<p>Babies with BPD are usually treated in a special intensive care unit (ICU) for newborns (neonatal ICU). They may spend several weeks or months in the hospital. Most children today who have BPD recover and many recover fully. However, some children who have had BPD remain prone to colds, flu, and other respiratory infections which may be unusually severe. The children may also grow more slowly than normal.</p>

Endometriosis



<p><b>Endometriosis:</b> In <A href="http://www.medicinenet.com/script/main/art.asp?articlekey=356">endometriosis</a>, cells that normally
grow inside the uterus (womb), instead grow outside the uterus.
<p><b>Summary:</b> Endometriosis is very common; the cause -- and why
some women have endometriosis and many others do not -- has not been
fully
fathomed, although there are several prevalent theories; most women
with
endometriosis have no symptoms; but pelvic pain during menstruation
or ovulation is a frequent symptom of endometriosis; endometriosis
may be suspected by during a physical examination; it is confirmed by
surgery, usually <A href="http://www.medicinenet.com/script/main/art.asp?articlekey=944">laparoscopy</a>; the available treatment includes
medication for pain, <A href="http://www.medicinenet.com/script/main/art.asp?articlekey=9817">hormone therapy</a>, and surgery.
<P>Endometrial cells line the uterus and are normally shed each month
during menstruation. When endometrial cells grow outside the uterus,
the cells implant. These implants occur most commonly within the
fallopian tubes and on the outside of the tubes and ovaries, the
outer surface of the uterus and intestines and anywhere on the
surface of the pelvic cavity. They can also be found, less often, on
the surface of the liver, in old surgery scars or, very rarely, in
the lung or brain.
<p>Endometrial implants respond to the hormones of the menstrual
cycle just as does the normal endometrium. The implants build up
during the month and then they break down and during menstruation
they bleed internally. Blood from the implants cannot leave the body
via the vagina (or by other exit). The internal bleeding, tissue
inflammation and, later, scarring cause the symptoms of endometriosis.
<P>Endometriosis occurs in the reproductive years. The average age at
diagnosis is 25-30. (It has been reported in girls as young as 11.)
<P>The central theory of the cause of endometriosis is retrograde
menstruation. Blood and endometrium tissue from the uterus back up
into the fallopian tubes and drip into the pelvic and abdominal
cavity. Retrograde menstruation may, it is thought, be due to a
defect in the uterus or it may be related to the way the uterus
contracts when it is expelling the menstrual tissue. Genetic factors
and the immune system probably are also important in determining
which women develop endometriosis and where the endometrial cells
implant.
<P>The common symptoms and signs are pain (usually pelvic) and
infertility. Pelvic pain usually occurs during or just before
menstruation and lessens after menstruation. Some women experience
pain or cramping with intercourse, bowel movements and/or urination.
Even a pelvic examination by a doctor can be painful.
<p>The intensity of pain may change from month to month and vary
greatly among women. Some women experience progressive worsening of
symptoms while others can have resolution without treatment.
<P>Endometriosis can cause infertility. When laparoscopy is done for
infertility studies, endometrial implants can be found in
some patients, many of whom may not have painful symptoms of
endometriosis. It believed that endometriosis bleeding, inflammation,
and scarring can cause distortion of the female reproductive organs
(such as obstruction of the fallopian tubes), resulting in
infertility. However, the severity of the disease is not necessarily
directly related to the degree of infertility.
<P>Other symptoms related to endometriosis include lower
<A href="http://www.medicinenet.com/script/main/art.asp?articlekey=1908"> abdominal pain</a>, <A href="http://www.medicinenet.com/script/main/art.asp?articlekey=1900">diarrhea</a> or <A href="http://www.medicinenet.com/script/main/art.asp?articlekey=331">constipation</a>, <A href="http://www.medicinenet.com/script/main/art.asp?articlekey=289">low back pain</a>, irregular or
heavy menstrual bleeding, or even blood in the urine. Rare symptoms
of endometriosis include chest pain or coughing blood due to
endometriosis in the lungs, <A href="http://www.medicinenet.com/script/main/art.asp?articlekey=20628">headache</a> and/or seizures due to
endometriosis in the brain.
</p>

<P>Endometriosis can become cancerous in less than 1% of women.
Most of the
cancers found with the condition, however, appear not to be
associated
with the implants, but rather occur independently of the
disease.
</p>

<p><b>How is endometriosis diagnosed?<br>
<br>
</b>Endometriosis can be suspected based on symptoms of pelvic
pain and
findings during physical examinations in the doctor's office.
Occasionally, the doctor can feel nodules (endometrial
implants) behind
the uterus and along the ligaments that attach to the pelvic
wall. At
other times, no nodules are felt, but the examination itself
causes
unusual pain or discomfort. Unfortunately, neither the symptoms
nor the
physical examinations can be relied upon to establish the
diagnosis of
endometriosis. Imaging studies, such as <A href="http://www.medicinenet.com/script/main/art.asp?articlekey=510">ultrasound</a>, can be
helpful in
studying the pelvis, but still cannot accurately diagnose
endometriosis.
Direct visual inspection and tissue biopsy of the implants are
necessary
for accurate diagnosis.</p>

<P>Currently, the only accurate way of diagnosing endometriosis
is at the
time of surgery (either by open standard laparotomy or
laparoscopy).
Laparoscopy is the most common surgical procedure for the
diagnosis of
endometriosis. Laparoscopy is a minor surgical procedure done
under
general anesthesia or in some cases local anesthesia. It is
usually
performed as an out-patient procedure (the patient going home
the same
day). Laparoscopy is performed by first inflating the abdomen
with carbon
dioxide through a small incision in the navel. A long, thin
instrument
(laparoscope) is then inserted into the inflated abdominal
cavity to
inspect the abdomen and pelvis. Endometrial implants can then
be directly
visualized. During laparoscopy, biopsies (removal of tiny
tissue samples
for examination under a microscope) can also be performed for a
diagnosis.
Sometimes biopsies obtained during laparoscopy show
endometriosis even
though no endometrial implants are visualized during
laparoscopy.
</p>

<P>Pelvic ultrasound and laparoscopy are also important in
excluding
malignancies (such as <A href="http://www.medicinenet.com/script/main/art.asp?articlekey=435">ovarian cancer</a>) that can mimic
endometriosis.
</p>

<P>Recently, there has been extensive research into blood testing
of
endometriosis. Unfortunately, the accuracy of these blood tests
has not
been firmly established.
</p>

<p><b>How is endometriosis treated?<br>
<br>
</b>Endometriosis can be treated with medications and/or surgery.
The goals
of endometriosis treatments include pain relieve, shrinkage or
ablation of
the implants, and enhancing fertility.</p>

<P>For pain relief, nonsteroid antiinflammatory drugs or <A
href="http://www.medicinenet.com/script/main/art.asp?articlekey=9520">NSAIDs</A>
(such as Motrin and Naprosyn) are given to help with menstrual cramping.
Narcotic
medications are sometimes prescribed when NSAIDs fail to
relieve the pain.
These analgesic medications have no effect on the endometrial
implants.
</p>

<P>Endometrial tissue (including implants) is affected by body's
estrogen
level. Estrogen is a female hormone made by the ovaries. When
estrogen
levels are low (as in <A href="http://www.medicinenet.com/script/main/art.asp?articlekey=2036">menopause</a> or after surgical removal of
ovaries),
endometrial tissue shrinks (perhaps even disappears), and
symptoms of
endometriosis ease.</p>

<P>Recently, a class of medications called GnRH analogs have been
developed
to treat endometriosis. These medications prevent the natural
releasing
hormones from being secreted, thereby blocking their
stimulating effect on
the gonads (ovaries in women). As a result, the normal
production of sex
hormones (estrogen in women) is reduced. One example of GnRH
analog is
<A href="http://www.medicinenet.com/script/main/art.asp?articlekey=838">nafarelin</a> (Synarel). When given by nasal spray for 6 months,
Synarel

Researchers
are trying to find ways of adding other hormones to counter the

used for

<P>There are other hormones available to treat endometriosis.
Some have
unpleasant side effects. Danazol (Danocrine) and

(Depo-Provera) are examples of medications that provide good
control of
symptoms but can cause severe side effects such as menstrual
changes,
<A href="http://www.medicinenet.com/script/main/art.asp?articlekey=9871">breast</a> soreness, <A href="http://www.medicinenet.com/script/main/art.asp?articlekey=1992">rash</a>, and fatigue and, therefore, are often
not suitable
for long-term use.</p>

alleviate
painful <A href="http://www.medicinenet.com/script/main/art.asp?articlekey=9814">menstrual cramps</a> and regulate periods in patients with

<P>Surgery can be an effective treatment for women with
moderate to severe endometriosis. Laparoscopic surgery is rapidly replacing open
laparotomy in diagnosing and treating endometriosis in the United States. With
<A href="http://www.medicinenet.com/script/main/art.asp?articlekey=944">laparoscopic surgery</A>





, the visible implants can be treated with
laser,
cautery, or other surgical instruments. Removal or destruction
of the
endometrial implants can relieve pain symptoms and allow
pregnancy to
occur in some women. However, in some cases, the condition may
be so
severe that major open surgery, removing the uterus and the
ovaries, is
required.</p>

<P>Even though medications and laparoscopic surgery are effective
in
decreasing implants and relieving pain, endometriosis and
symptoms can
recur. For example, after completing 6 months of treatment with Synarel,
60% of the patients became symptom free, 32% experienced mild
symptoms,
and the remaining 8% had moderate to severe symptoms. Of the
patients who
had complete relief of symptoms, 50% of them remained free of
symptoms 6
months after stopping treatment, 33% developed mild symptoms
and 17%
developed severe symptoms. Even after open radical surgery with
removal of
the uterus and the ovaries, severe endometriosis can reactivate,
especially if hormone replacement is instituted shortly after
surgery.
</p>

<P>A great deal of effort is currently being focused on
diagnosing and
treating endometriosis. Newer methods of laparoscopy with
smaller
incisions and instruments are being developed. Newer drugs and
different
drug combinations are being explored. A new treatment based on
strengthening the immune system (immunotherapy) may be helpful
in women in
whom there is felt to be an immune system dysfunction. Newer,
more
sensitive methods of radiological imaging are being examined for
diagnostic purposes. Certain proteins in the saliva are being
researched
as a method of diagnosis. Overall, endometriosis is a condition
that can
be treated successfully in the majority of women, allowing them
to achieve
pain relief, pregnancy, or both.
</p>A benign condition in which
tissue that looks like endometrial tissue grows in abnormal places, most often
in the abdomen. Although most women with endometriosis have no symptoms, pelvic
pain during menstruation or ovulation can be a symptom of endometriosis.
Endometriosis can also be suspected by a doctor during a physical examination
and confirmed by surgery, usually laparoscopy. Treatment options include
medications for pain, hormone therapy, and surgery. The word "endometriosis" was
coined in 1927 by John Sampson who suggested that menstrual blood backed up from
the uterus carrying endometrial cells through the fallopian tubes into the
abdominal cavity so that these cells could implant, for example, on the ovaries,
ureters (the tubes coming from the kidney to the bladder), and the top of the
vagina (called the cul-de-sac).

is very common. A stroke involving the base of the brain can affect
balance, vision, swallowing, breathing and even unconsciousness.
<P>A stroke is <b>a medical emergency</b>. Anyone suspected
of having a stroke should be taken immediately to a medical facility
for diagnosis and treatment.
<p>The <u>causes of stroke</u>: An artery to the brain may be blocked
by a clot (thrombosis) which typically occurs in a blood vessel that
has previously been narrowed due to atherosclerosis ("hardening of
the artery"). When a blood clot or a piece of an atherosclerotic
plaque (a cholesterol and calcium deposit on the wall of the artery)
breaks loose, it can travel through the circulation and lodge in an
artery of the brain, plugging it up and stopping the flow of blood;
this is referred to as an embolic stroke. A blood clot can form in a
chamber of the heart when the heart beats irregularly, as in atrial
fibrillation; such clots usually stay attached to the inner lining of
the heart but they may break off, travel through the blood stream,
form a plug (embolus) in a brain artery and cause a stroke. A
cerebral hemorrhage (bleeding in the brain), as from an aneurysm
(a widening and weakening) of a blood vessel in the brain, also
causes stroke.</p>
<P>The <u>diagnosis of stroke</u> involves a medical history and a
physical examination. Tests are done to search for treatable causes
of a stroke and help prevent further brain damage. A CAT scan (a
special X-ray study) of the brain is often done to show bleeding into
the brain; this is treated differently than a stroke caused by lack
of blood supply. A CAT scan also can rule out some other conditions
that may mimic a stroke. A soundwave of the heart (echocardiogram)
may be done to look for a source of blood clots in the heart.
Narrowing of the carotid artery (the main artery that supplies blood
to each side of the brain) in the neck can be seen with a soundwave
test called a carotid ultrasound. Blood tests are done to look for
signs of inflammation which can suggest inflamed arteries. Certain
blood proteins are tested that can increase the chance of stroke by
thickening the blood.
<p><u>Stroke look-alikes</u>: Just because a person has slurred speech
or weakness on one side of the body does not necessarily mean that
person has had a stroke. There are many other nervous system
disorders
that can mimic a stroke including a brain tumor, a subdural
hematoma (a collection of blood between the brain and the skull) or a
brain abscess (a pool of pus in the brain caused by bacteria or
a fungus). Virus infection of the brain (viral encephalitis) can
cause symptoms similar to those of
a stroke, as can an overdose of certain medications. <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=339">Dehydration</a> or
an imbalance of sodium, calcium, or glucose can cause neurologic
abnormalities similar to a stroke.
<P><u>Treatment of a stroke</u>: Early use of anticoagulants to
minimize blood clotting has value in some patients. Treatment of
blood pressure that is too high or too low may be necessary.
(Lowering elevated blood pressure into the normal range is
no longer recommended during the first few days following a stroke
since this may further reduce blood flow through narrowed arteries
and make the stroke worse.) The blood sugar glucose in diabetics is
often quite high after a stroke; controlling the glucose level may
minimize the size of a stroke. Drugs that can dissolve blood clots
may be useful in stroke treatment. Oxygen is given as needed. New
medications that can help oxygen-starved brain cells survive while
circulation is reestablished are being developed.</p>
<P><u>Rehabilitation</u>: When a patient is no longer acutely ill
after a stroke, the aim turns to maximizing the patient's
functional abilities. This can be done in an inpatient rehabilitation
hospital or in a special area of a general hospital and in a nursing
facility. The rehabilitation process can involve speech therapy to
relearn talking and swallowing, occupational therapy for regaining
dexterity of the arms and hands, physical therapy for improving
strength and walking, etc. The goal is for the patient to resume as
many of their pre-stroke activities as possible.</p>
<p><u>Footnote</u>: The term "stroke" reflects the belief among the
