malformations in Down syndrome.</p>

<p>Down syndrome is also associated with a number of major
malformations. For example, approximately a half of Down syndrome
children are born with a heart defect, most often a hole between the
two sides of the heart. For another example, Hirschprung's disease
(congenital aganglionic megacolon) which can cause intestinal
obstruction occurs more frequently in children with Down syndrome
than in other children.</p>

<p>The intellectual handicaps in Down syndrome are often the most
important problem. These handicaps may not be evident in early
infancy. However, they tend to become increasingly noticeable later
in infancy and during childhood as developmental delay. In adults
with Down syndrome, the intellectual handicap is manifest as mental
retardation.

<p>Very few adults with Down
syndrome can lead independent lives because of their mental
retardation. It was once thought that nearly all adults with Down
syndrome
developed Alzheimer's disease (<a href="http://www.medicinenet.com/script/main/art.asp?articlekey=9090">dementia</a>) so that on top of their
mental handicap most people with Down syndrome were slated for
premature senility. However, it now appears that a much lower

damage to the brain caused by <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=489">stroke</a> or injury, and there may be an
actual shrinkage of brain substance. "Hydrocephalus ex-vacuo" is
essentially only hydrocephalus by default; the CSF pressure itself is
normal.
<p>Normal pressure hydrocephalus (NPH) can also occur due to a
gradual blockage of the CSF drainage pathways in the brain. Although
the ventricles enlarge, the pressure of the CSF remains within normal
range. NPH is characterized by memory loss (<a href="http://www.medicinenet.com/script/main/art.asp?articlekey=9090">dementia</a>), gait disorder,
<a href="http://www.medicinenet.com/script/main/art.asp?articlekey=7999"> urinary incontinence</a> and a general slowing of activity.
<p>In old age or persons with Alzheimer's disease, the entire brain
may shrink and the CSF fills up the space created by the shrinkage.
This is not due to hydrocephalus.
<p>The diagnostic signs and symptoms of hydrocephalus depend upon the
age of the person:<ul>
<li>In infants the most obvious sign of hydrocephalus is
usually an abnormally large head. (That is one reason a baby's head
should be measured at every well-baby visit). Symptoms of
hydrocephalus in an infant may include vomiting, sleepiness,
irritability, an inability to look upwards, and seizures.</li>
<li>In older children and adults there is no head enlargement
from hydrocephalus, but symptoms may include headache, nausea,
vomiting and, sometimes, blurred vision. There may be problems with
balance, delayed development in walking or talking, and poor
coordination.</li>
</ul>
Irritability, fatigue, seizures, and personality changes such as an
inability to concentrate or remember things may also develop.
Drowsiness and double vision are common symptoms as hydrocephalus
progresses.
<p>Treatment of hydrocephalus involves the insertion of a shunt to
let the excess
fluid exit and relieve the pressure on the brain. The shunt is a
flexible, plastic tube with a oneway valve. The shunt is inserted
into the ventricular system of the brain to divert the flow of CSF
into another area of the body, where the CSF can drain and be
absorbed into the bloodstream.
<p>The prognosis (outlook) with hydrocephalus depends the cause and
the timing of the diagnosis and treatment. Many children treated for
hydrocephalus are able to lead normal lives with few, if any,
limitations. In some cases, cognitive impairments in language and
non-language functions may occur. Problems with shunts such as
infection or malfunction require revision of the shunt.

test.</P>
<P>The PSA value used most frequently as the highest normal level is
4 ng/mL (nanograms per milliliter). However, since the prostate gland
generally increases in size and produces more PSA with increasing
age, it is normal to have lower levels in young men and higher levels
in older men. Age-specific PSA levels are as follows (age group,
upper normal): (40 - 49, 2.5), (50 - 59, 3.5), (60 - 69, 4.5), (70 -
79, 6.5). The use of age-specific PSA ranges for the detection of
<a href="http://www.medicinenet.com/script/main/art.asp?articlekey=456"> prostate cancer</a> is controversial. Not all studies have agreed that
this is better than simply using a level of 4 ng/mL as the highest
normal value.</P>
<P>The PSA test is used in several distinctly different ways with respect to detecting <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=456">prostate cancer</a>. It can be used in men who are not known to have the disease as a screening test. It can be used to gauge the aggressivity of prostate cancer. And it can be used in men
with prostate cancer to monitoring their disease.</p>
<ul>
<li>Screening test: An abnormal PSA result usually requires additional testing. Levels above 4 ng/mL but less than 10 ng/mL are considered suspicious. However, most men who have this level of PSA do
not have prostate cancer. As the PSA levels increase above 10 ng/mL, the probability of prostate cancer increases dramatically.</li><br><br>
<li>Gauge of aggressivity: Men whose PSA level rises rapidly (more than 2 ng per milliliter) during the year before the diagnosis of prostate cancer have a higher risk of dying from prostate cancer despite undergoing radical prostatectomy (surgery that removes the entire prostate gland plus some surrounding tissue).</li><br><br>
<li>Monitoring test: An abnormal PSA result following therapy may indicate recurrence of the prostate cancer.</li></ul>
<P>PSA is not specific to prostate cancer. Other diseases can cause
an elevated PSA. The most frequent is benign prostatic hypertrophy
(BPH), an increase in the size of the prostate that typically occurs
with aging. Infection of the prostate gland (<a href="http://www.medicinenet.com/script/main/art.asp?articlekey=457">prostatitis</a>) is another
relatively common cause of an elevated PSA. Other confounding
conditions that can increase PSA include ischemia or infarction,
urethral instrumentation, and urinary retention, and prostate
biopsy.</p>
<P>The PSA test has other limitations. A small proportion of prostate
cancers do not produce a detectable increase in blood PSA, even with
advanced disease. Many early cancers will also not produce enough PSA
to cause a significantly abnormal blood level. It is therefore
important not to rely only on blood PSA testing. The most useful
additional test is a physical prostate exam known as the digital
rectal exam (DRE).</P>

Hypospadias



<p><b>Hypospadias:</b> A <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=11042">birth defect</a> of the <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=4827">penis</a> involving

<p>Hypospadias is relatively common, affecting approximately 1 in every 500 newborn boys. Hypospadias can occur as an isolated birth defect in an otherwise normal child or it can be part of a multiple malformation syndrome. There is no association between the mother's use of loratadine (Claritin) in early pregnancy and the occurrence of hypospadias in her son.
<p> In mild type (first-degree) hypospadias, the urethral opening
is below the tip but still nearby (on what is called the
<a href="http://www.medicinenet.com/script/main/art.asp?articlekey=11380">glans</a>). In moderate (second-degree) hypospadias, the
urethra opens closer to the body on the underside of the
shaft of the penis. In severe (third-degree) hypospadias,
the urethal opening is below the penis on the skin and this
is called a <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=11877">perineal</a> hypospadias.</p>
<p>In a child with hypospadias, the urine comes from an opening that is on the underside of the penis or below the penis. With
moderate-to-severe degrees of hypospadias, the boy will
therefore usually sit to urinate. Some cases of
first-degree hypospadias are so mild as to not require
treatment.
<p>If the hypospadias needs to be repaired, this is done
by surgery. The aim is to repair and reconstruct the urethra.
Depending on the degree of the hypospadias, the
surgery can range from relatively simple to very
challenging.
<p>The opposite of hypospadias is <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=3289">epispadias</a>, a less common birth defect in which the urethra opens prematurely on the upperside of the penis or above it.</P>

Eczema vaccinatum




<p><b>Eczema vaccinatum:</b> A common concern with smallpox vaccination involving the implantation of the vaccinia virus from the vaccination into the skin of a person with eczema (<A href="http://www.medicinenet.com/script/main/art.asp?articlekey=353">atopic dermatitis</a>), sometimes with a fatal outcome. Disrupted skin permits viral implantation. Once the virus is implanted, it spreads from cell to cell producing extensive lesions. The skin lesions appear identical to a primary smallpox vaccination. Confluent lesions often cover the entire face, the crook of the elbow in the antecubital fossa, and behind the knee in the popliteal fossa. Viremia (viral spread through the bloodstream) may also occur allowing for the spread of virus to other parts of the body, including skin that is not affected by eczema. Bacterial and fungal invasion may occur as a late stage of untreated eczema vaccinatum.
<p>It is estimated that there are 27 million individuals in the US alone who have <A href="http://www.medicinenet.com/script/main/art.asp?articlekey=353">atopic dermatitis</a>, many of whom would be susceptible to eczema vaccinatum if vaccinated or in contact with a vaccine. Further, it appears that even healed skin is not normal and eczema vaccinatum has occurred in the skin of such individuals at the sites of prior florid eczema.
<p>The key to treatment is the use of Vaccinia Immune Globulin (VIG). With early recognition and appropriate use of VIG, mortality (the risk of death) can be reduced to zero, and morbidity alleviated. Bacterial and fungal invasion require antibiotic therapy.
<p>To prevent eczema vaccinatum, vaccination should not be performed in patients with eczema and they should not be in contact with vaccines, if smallpox is not an immediate risk. However, if there is smallpox in the community with potential exposure, or if the patient is a household contact of a case, then vaccination must be performed.</p>

Untreated, it usually leads to death by age 10 from liver failure or hepatocarcinoma (<a href="http://www.medicinenet.com/script/main/art.asp?articlekey=13931">cancer</a> of the liver).
<p>A diet with a special formula that lacks the amino acids phenylalanine and tyrosine can slow the inevitable progression of the disorder. Liver transplantation provides a better long-term outcome than diet alone. A drug called NTBC that inhibits the tyrosine metabolic pathway can reverse the symptoms of tyrosinemia and help the liver and kidney tissue to return to normal.
<p>The usual treatment of tyrosinemia today is NTBC plus diet. This permits greater than 90% survival. Infants with tyrosinemia and cancer of the liver need a liver transplant to survive.
<p>Tyrosinemia type II is due to deficiency of the enzyme tyrosine transaminase. It is characterized by the deposition of crystals of tyrosine in the skin and eyes. Thickened areas (keratoses) on the palms and soles become painful and ulcers develop in the cornea. There is often mental retardation in tyrosinemia type II.
