<p>People with galactosemia are unable to metabolize the simple sugar galactose. Galactose makes up half of the sugar called lactose that is found in milk. Lactose is called a disaccharide, di meaning 2, since lactose is made up of two sugars, galactose and <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=3608">glucose</a>, bound together. If an infant with galactosemia is given milk, galactose builds up in the infants system causing damage to the liver, brain, kidneys and eyes. Individuals with galactosemis cannot tolerate any form of milk (human or otherwise) or any other galactose-containing food. Exposure to milk products will result in liver damage, mental retardation, <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=2645">cataract</a> formation, and kidney failure.

More than 50 different mutations in transthyretin are known in connection with this type of amyloidosis.</p>
<P>There are also other proteins which, when mutated, can cause familial amyloidosis, but these situations are quite rare. (These proteins include apolipoprotein A1, gelsolin, fibrinogen, and lysozyme.)</P>
<P>In hereditary amyloidosis, the nervous system and gastrointestinal tract are often involved. This can cause numbness and tingling in the arms and legs, <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=8004">dizziness</a> upon standing, and <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=1900">diarrhea</a>. Each family with hereditary amyloidosis has its own pattern of organ involvement and associated symptoms.</p>
<P>The mode of transmission in hereditary amyloidosis is autosomal dominant, which means that if a male or female has this type of amyloidosis each of their children has a 1-in-2 (50%) chance of inheriting the same mutation and of having the same disease. <P> The only treatment that offers a potential cure for hereditary amyloidosis is liver transplantation. <p><b>Tyrosinemia:</b> A genetic disorder involving the metabolism of the amino acid tyrosine characterized by abnormally high levels of tyrosine in blood (hypertyrosinemia) and urine (tyrosinuria). There are several different forms of tyrosinemia.
<p>The classic form, tyrosinemia type I, is due to deficiency of an enzyme called fumarylacetoacetic hydrolase, the last enzyme in the tyrosine pathway. Although generally rare, its incidence is greatly elevated in Norway, Finland, and Quebec.
<p>Tyrosinemia type I is inherited as an autosomal recessive disorder and causes <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=322">cirrhosis of the liver</a> before 6 months of age and, untreated, leads to death from liver failure. A more chronic form of the disease is characterized by progressive cirrhosis of the liver, a renal syndrome (with loss of phosphate into the urine causing rickets of renal origin and growth failure) and recurring neurologic crises. Untreated, it usually leads to death by age 10 from liver failure or hepatocarcinoma (<a href="http://www.medicinenet.com/script/main/art.asp?articlekey=13931">cancer</a> of the liver).
<p>A diet with a special formula that lacks the amino acids phenylalanine and tyrosine can slow the inevitable progression of the disorder. Liver transplantation provides a better long-term outcome than diet alone. A drug called NTBC that inhibits the tyrosine metabolic pathway can reverse the symptoms of tyrosinemia and help the liver and kidney tissue to return to normal.
<p>The usual treatment of tyrosinemia today is NTBC plus diet. This permits greater than 90% survival. Infants with tyrosinemia and cancer of the liver need a liver transplant to survive.
<p>Tyrosinemia type II is due to deficiency of the enzyme tyrosine transaminase. It is characterized by the deposition of crystals of tyrosine in the skin and eyes. Thickened areas (keratoses) on the palms and soles become painful and ulcers develop in the cornea. There is often mental retardation in tyrosinemia type II. Hepatocellular carcinoma

<p>Primary liver cancer (hepatocellular carcinoma) is the most common cancer in some parts of the world. It is still relatively uncommon in the US but its <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=11516">incidence</a> is rising, principally in relation to the spread of <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=3708">hepatitis B</a> and <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=3710">hepatitis C</a>. People who have a disease of the liver called <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=2740">cirrhosis</a> are also more likely to get adult primary liver cancer.

The patient lies on a table and the medication, adenosine, will be given through the IV. The heart pumps faster and more strongly. The tracer is then given through the IV.
This part of the scan lasts about 25 minutes.
<p>A second set of pictures is taken approximately 3 hours after the injection. The patient should not eat but can drink fluids that do not contain caffeine. The entire test takes approximately 5 to 6 hours.
<p>To prepare for the scan, the patient should not drink or eat anything containing caffeine for 24 hours before the test (this includes decaf coffee and chocolate) and should not eat or drink after midnight and not smoke on the day of the test. Most medications are taken as usual; however, some medications (e.g., <A href="http://www.medicinenet.com/script/main/art.asp?articlekey=844">theophylline</a>, Persantine, and trental) can interfere with the adenosine scan. One should seek special instructions from the doctor or nurse, as should all diabetics.
<p>The amount of radiation from the scan is no more than from other x-ray tests. If there is any chance a woman might be pregnant, she should notify the doctor or nurse before the test is done. If a patient has <A href="http://www.medicinenet.com/script/main/art.asp?articlekey=284">asthma</a> or other lung problems, he or she should also notify the doctor or nurse before the test is done. feeling of a foreign body in the throat (the globus phenomenon).
<P>Treatment includes:<ul>
<li>Antacids neutralize stomach acid and give immediate relief.
Popular choices include sodium bicarbonate (Alka Seltzer), calcium
carbonate (Tums, Rolaids, Alka-Mints), and aluminum and magnesium
antacids (Maalox, Mylanta, Riopan, Gaviscon), all best used 30-60
minutes after each meal and at bedtime.</LI>

<LI>Give the stomach several hours to empty before you go to
bed. Try not to eat after your evening meal. </LI>

<LI>Try eating a diet that is high-protein, high-carbohydrate,
and low-fat. </LI>

<LI>Avoid alcohol, caffeinated beverages, and tobacco. </LI>

Carbon monoxide tends to build up above the water near the water platform. The amount of carbon monoxide that can build up in the air space beneath the stern deck on houseboats can be deadly within seconds to minutes and can also reach life-threatening concentrations on and near the swim deck.</p>
<p>
The most common symptoms of carbon monoxide poisoning are <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=20628">headache</a>, dizziness, weakness, nausea, vomiting, chest pain, and confusion. High levels of carbon monoxide can cause loss of consciousness and death, or can cause someone to pass out and fall into the water and drown. Unless suspected, carbon monoxide poisoning can be difficult to diagnose because the symptoms seem like other illnesses. People who are sleeping or intoxicated can die from carbon monoxide poisoning before ever experiencing symptoms.</p>
<p>
If you think someone on a houseboat has carbon monoxide poisoning, move him or her to fresh air quickly AND contact the nearest emergency services.</p>
<p>To prevent carbon monoxide poisoning on a houseboat in the first place, do the following:
<ul>
<li>Install and maintain a working carbon monoxide detector inside the houseboat. (Remember, this wont alert people to dangerous levels of carbon monoxide at locations outside the houseboat cabin, such as the swim deck).
<li>Make sure that all fuel-burning engines and appliances are properly installed, maintained, and operated.
<li>Educate all passengers about the signs, symptoms, and treatment of carbon monoxide poisoning, especially if the houseboat has a rear swim deck or water platform.
<li>Watch children closely when they are playing on rear swim decks or water platforms for signs of carbon monoxide poisoning.
<li>Swim and enjoy other activities away from areas where gasoline-powered engines vent their exhaust.
</ul>
<p>Carbon monoxide from exhaust pipes of inboard engines, outboard engines, and generators builds up inside and outside any boat in areas near the exhaust vents. From 1994 to 2004, 31 deaths were reported to the US Coast Guard involving all types of recreational boats. The Coast Guard reminds boaters to be aware of the following:
<ul>
<li>Blocking exhaust outlets can cause carbon monoxide to build up in the cabin and cockpit areas even when hatches, windows, portholes, and doors are closed.
<li>Exhaust from another vessel that is docked, beached, or anchored beside your boat can send carbon monoxide into the cabin and cockpit of your boat. Your boat should always be at least 20 feet from the nearest boat that is running a generator or engine.
<li>Slow speeds or idling in the water can cause carbon monoxide to build up in the cabin, cockpit, bridge, and aft deck, even in an open area. Wind entering from the aft section of the boat can also increase this build up of carbon monoxide.
<li>The "station wagon effect" (back drafting) can cause carbon monoxide to build up inside the cabin, cockpit, and bridge when operating the boat at a high bow angle, with improper or heavy loading, or if there is an opening that draws in exhaust.
</ul>
