A major feature of the Landau-Kleffner syndrome (LKS) is the gradual or sudden loss of the ability to understand and use spoken language. All children with LKS have abnormal electrical brain waves that can be documented by an electroencephalogram (EEG), a recording of the electric activity of the brain. Approximately 80 percent of the children with LKS have one or more epileptic seizures that usually occur at night. Behavioral disorders such as hyperactivity, aggressiveness and depression can also accompany this disorder.

LKS occurs most frequently in normally developing children who are between 3 and 7 years of age. For no apparent reason, these children begin having trouble understanding what is said to them. Doctors often refer to this problem as auditory agnosia or "word deafness." The auditory agnosia may occur slowly or very quickly. Parents often think that the child is developing a hearing problem or has become suddenly deaf. Hearing tests, however, show normal hearing. Children may also appear to be autistic or developmentally delayed.

The inability to understand language eventually affects the child's spoken language which may progress to a complete loss of the ability to speak (mutism). Children who have learned to read and write before the onset of auditory agnosia can often continue communicating through written language. Some children develop a type of gestural communication or sign-like language. The communication problems may lead to behavioral or psychological problems. Intelligence usually appears to be unaffected.

The loss of language may be preceded by an epileptic seizure that usually occurs at night. At some time, 80 percent of children with LKS have one or more seizures. The seizures usually stop by the time the child becomes a teenager. All LKS children have abnormal electrical brain activity on both the right and left sides of their brains.

The cause of LKS is unknown. Some experts think there is more than one cause for this disorder. All of the children with LKS appear to be perfectly normal until their first seizure or the start of language problems. There have been no reports of children who have a family history of LKS. Therefore, LKS is not likely to be an inherited disorder.

There have not been many long-term follow-up studies of children with LKS. This lack of evidence, along with the wide range of differences among affected children, makes it impossible to predict the outcome of this disorder. Complete language recovery has been reported; however, language problems usually continue into adulthood. The continued language problems can range from difficulty following simple commands to no verbal communication. If recovery takes place, it can occur within days or years. So far, no relationship has been found between the extent of the language impairment, the presence or absence of seizures and the amount of language recovery. Generally, the earlier the disorder begins, the poorer the language recovery. Most children with LKS outgrow the seizures, and electrical brain activity on the EEG usually returns to normal by age 15.

Medication to control the seizures and abnormal brain wave activity (anticonvulsants) usually has very little effect on language ability. Corticosteroid therapy has improved the language ability of some children. Sign language instruction has benefited others.

LKS may also be called infantile acquired aphasia, acquired epileptic aphasia or aphasia with convulsive disorder. The syndrome was first described in 1957 by Drs. William M. Landau and Frank R. Kleffner, who identified 6 children with the disorder.

Fragile X syndrome

Fragile X syndrome: One of the most common causes of inherited mental retardation and neuropsychiatric disease in human beings, affects as many as one in 2000 males and one in 4000 females. The syndrome is also known as FRAXA (the fragile X chromosome itself) and as the Martin-Bell syndrome. However, the preferred name is fragile X syndrome.
The characteristic features of the fragile X
syndrome in boys include prominent or long ears, a long face, delayed
speech, large testes (macroorchidism), hyperactivity, tactile
defensiveness, gross motor delays, and autistic-like behaviors.
Much less is known about girls with fragile X syndrome. Only about half of all females who carry the genetic mutation have symptoms themselves.
Of those, half are of normal intelligence, and only one-fourth have
an IQ under seventy. Few fragile X girls have autistic symptoms,
although they tend to be shy and quiet.
Fragile X syndrome is due to a dynamic mutation (a trinucleotide repeat) at an inherited fragile site on the X chromosome, and so is an X-linked disorder. Because the
mutation is dynamic, it can change in length and hence in severity
from generation to generation, from person to person, and even within
a given person.
The diagnosis of the fragile X syndrome is confirmed by the detection of an increased number of CGG trinucleotide repeats (over 230) in the FMR1 gene, usually with aberrant methylation of the FMR1 gene. The increased trinucleotide repeats and methylation changes in FMR1 can be detected by molecular genetic testing.
Trinucleotide repeat expansion of at least three genes located on the X chromosome has now been associated with the formation of "fragile sites." Trinucleotide expansions within two of these
genes, FRAXA (within the FMR1 gene) and FRAXE (within the FMR2 gene), are associated with mental retardation, while that at a third, FRAXF (not yet associated with a specific gene) is not. Mutations in the FMR1 gene, for example, produce expansion of CGG repeats in the 5' untranslated region of the gene and lead to a severely disabling neurodevelopmental disorder. Such expansion leads to physical, neurocognitive, and emotional characteristics linked to, but not exclusively determined by, alterations in the FMR1 gene or
the level of its protein, FMRP.

Approximately 15-25% of individuals with fragile X syndrome also are diagnosed with mild to moderate <A href="http://www.medicinenet.com/script/main/art.asp?articlekey=287">autism</a> and autism spectrum disorders. Expression of the protein FMRP and autistic status appear to be associated with developmental outcome. Other clinical abnormalities associated with fragile X syndrome include <A href="http://www.medicinenet.com/script/main/art.asp?articlekey=258">attention deficit hyperactivity disorder</a> (ADHD) and anxiety
disorders.

The laboratory methods for the prenatal diagnosis of gene expansion of FRAXA, associated with FMR1 and its encoded protein, FMRP, are accurate, sensitive, and relatively inexpensive. For such methods to be applied as general screening techniques, however, more knowledge (about prevalence, risk factors, and nature of high repeat alleles) needs to be obtained. Because different human populations may not be equivalent with respect to fragile X expansion, consideration of ethical issues related to variability of phenotype, the possibility of mislabeling, and the value of screening if there is no definitive therapy, is required.

Although FMR1 is subject to X inactivation, abnormal methylation appears to contribute to the phenotype observed in mosaic patients. These individuals experience upregulation of the FMR1 gene. Further, expanded repeats appear to be abnormally methylated from the start, although the mechanism by which such abnormal methylation occurs remains unknown. Expanded FMR1 genes are also "silenced" through a process of deacetylation.

FMRP is found in both nucleus and cytoplasm, where it binds with mRNAs associated with ribonucleoproteins (RNPs) specifically associated with polyribosomes. In neurons, FMRP-associated RNPs are located in the cell body, as well as in the dendrites, at the base of dendritic spines. Thus, FMRP may play a role in synaptic function and plasticity.

Individuals with fragile X syndrome also exhibit neuroendocrinologic and reproductive disorders. Macroorchidism occurs in males with overt fragile X syndrome. Premature ovarian failure occurs in females who do not have the number of CGG repeats to produce overt fragile X syndrome, but have an expanded number of repeats with few or no apparent neurological symptoms
(premutation carriers).

Variation in the cognitive and behavioral phenotype of the fragile X syndrome has been demonstrated in intellectual functioning, learning disability, executive function, attention, hyperactivity, <A href="http://www.medicinenet.com/script/main/art.asp?articlekey=342">depression</a>, anxiety, and autistic behaviors. The explanation for this variation in phenotypic expression may depend on understanding the role of genetics and brain development in cognition and behavior.

For treatment modalities to be instituted for the fragile X syndrome, suppression of expansion, restoration of expression, small molecule agents, and gene replacement will need

Exercise-induced bronchospasm is common. People with chronic
asthma can develop symptoms whenever they are exposed to a "trigger"
of the asthma, such as a virus, pollen, dust, or cigarette smoke.
About 80 to 90 percent of people who have chronic asthma have
exercise-induced asthma. And about 35 to 40 percent of people with
seasonal allergies also have exercise-induced asthma and symptoms
worsen during the spring and fall.

Acute attacks of exercise-induced asthma can often be avoided by
warming up before strenuous activity. Of note, cold dry air is
believed to trigger exercise-induced asthma. Exercising outdoors in
the winter or mouth breathing can set off the asthma attack. Some
doctors recommend indoor swimming as an ideal form of exercise
because the warm, humid air keeps the airways from drying and
cooling.

Exercise-induced asthma is monitored using a peak-flow meter. This
hand-held device measures air flow (how fast air is blown out of the

resembling an insect bite. It can be red, pink, or purple in color.
The bump (<a href="http://www.medicinenet.com/script/main/art.asp?articlekey=4576">nodule</a>) usually appears on the finger, hand, or arm where
the fungus first entered through a break on the skin. This is
followed by one or more additional bumps or nodules which
open. They may look like boils. Eventually, the bumps turn into open
hollowed-out sores (ulcerations), which are very slow to heal. The
infection can also spread to other areas of the body.

The large majority of infections with sporotrichosis are limited
to the skin (localized). Sporotrichosis infection can spread through
the blood to other areas (disseminated), but this is relatively rare.
For example, infection of the joints, lungs, eye, and the
<a href="http://www.medicinenet.com/script/main/art.asp?articlekey=3579"> genitourinary</a> and central nervous system (brain and spine) can result. Usually,
disseminated sporotrichosis occurs in people who have problems with

Tetraethyl lead

Tetraethyl lead: An
anti-knock compound added to motor fuel. Also known as tetraethylplumbane,
tetraethyl lead has highly adverse effects on human health. It causes <A href="http://www.medicinenet.com/script/main/art.asp?articlekey=6234">lead poisoning</a>.
The History of Tetraethyl Lead
In 1921 three General Motors (GM) engineers -- Charles
Kettering, Thomas Midgeley, and Thomas Boyd -- reported success with adding
tetraethyl lead to improve engine performance and reduce engine knock. Through
the Ethyl corporation, then a GM subsidiary, GM quickly began touting this lead
compound as the virtual savior of the American automobile industry. The
discovery was indeed extremely important. It paved the way for the development
of the high-power, high-compression internal combustion engines.
The first danger sign was the mysterious illness that
forced Thomas Midgeley to spend weeks convalescing in the winter of 1923.
Midgeley had been experimenting rather recklessly with the various methods of
manufacturing tetraethyl lead, and he did not at first realize just how
dangerous the substance was in its concentrated liquid state. The deadliness of tetraethyl lead was sadly confirmed in the summer of 1924. Workers engaged in producing the additive fell sick and died at several refineries in New Jersey and Ohio. Banner headlines greeted each new fatality until a total of 15 workers had lost their minds and then their lives.
In 1925, the US <A href="http://www.medicinenet.com/script/main/art.asp?articlekey=11998">Surgeon General</a> temporarily suspended the production and sale of <A href="http://www.medicinenet.com/script/main/art.asp?articlekey=40272">leaded gasoline</a>. He appointed a panel of experts to investigate the recent fatalities that had "occurred in the manufacture and mixing of the concentrated tetraethyl lead." The panel was also asked to weigh "the possible danger" that might arise "from...wide distribution of a lead compound" through its sale as a gasoline additive.
Industry dominated the Surgeon General's investigatory
committee, which included only one genuine environmental visionary, Dr. Alice
Hamilton of Harvard University. The Coolidge Administration gave the panel just
seven months to design, run, and analyze its tests. The committee's final
report, published in June 1926, complained of the time constraints under which
it had been forced to operate. Seven months was "not sufficient," argued the
panel, "to produce detectable symptoms of lead poisoning" in experimental subjects because of the very slow <A href="http://www.medicinenet.com/script/main/art.asp?articlekey=3589">gestation</a> of
that toxicological syndrome.
Nevertheless, the Surgeon General's panel ruled that
there were "no good grounds for prohibiting the use of ethyl gasoline...as a
motor fuel, provided that its distribution and use are controlled by proper
regulations." The coming decades of <a href="http://www.medicinenet.com/script/main/art.asp?articlekey=342">Depression</a>, total war, and post-war boom
were hardly conducive to the implementation of "proper regulations" for leaded
gasoline. Indeed, no compulsory standards were set for the industry until the early 1970s when EPA began its long, hard struggle to phase down lead levels in US gasoline.
One saturnine prophecy marred the otherwise <A href="http://www.medicinenet.com/script/main/art.asp?articlekey=9341">sanguine</a> 1926 report to the Surgeon
General. By 1985 these words were to reverberate with particular resonance down
the corridors of time: "It remains possible that, if the use of leaded gasolines
becomes widespread, conditions may arise very different from those studied by us
which would render its use more of a hazard than would appear to be the case
from this investigation. Longer experience may show that even such slight
storage of lead as was observed [among human guinea pigs] in these [1925]
studies may lead eventually to recognizable lead poisoning or to chronic
degenerative diseases of a less obvious character. In view of such possibilities
the committee feels that the investigation begun under their direction must not
be allowed to lapse.... With the experience obtained and the exact methods now
available, it should be possible to follow closely the outcome of a more
extended use of this fuel and to determine whether or not it may constitute a
menace to the health of the general public after prolonged use or under
conditions not now foreseen.... The vast increase in the number of automobiles
throughout the country makes the study of all such questions a matter of real
importance from the standpoint of public health." Needless to say, this advice fell on deaf ears.
In 1927 the Surgeon General set a voluntary standard for the oil industry to follow in mixing tetraethyl lead with gasoline. This standard -- 3 cubic centimeters per gallon (cc/g) -- corresponded to the maximum then in use among refiners, and thus imposed no real restraint. Even without prodding, however, the industry did take giant strides toward instituting safer working conditions in oil refineries, thereby protecting individual laborers in the microcosm of the workplace.
Three decades later, the Surgeon General actually raised
the lead standard to 4 cc/g (equivalent of 4.23 grams per gallon). This
voluntary standard once again represented the outside range of industry practice. Nevertheless, the Surgeon General concluded in 1958 that a loosening of the voluntary standard posed no threat to the health of the average American: "During the past 11 years, during which the greatest expansion of tetraethyl lead has occurred, there has been no sign that the average individual in the U.S. has sustained any measurable increase in the concentration of lead in his <A href="http://www.medicinenet.com/script/main/art.asp?articlekey=2483">blood</a> or in the daily output of lead in his <A href="http://www.medicinenet.com/script/main/art.asp?articlekey=5915">urine</a>."
The actual industry average during the 1950s and the
1960s hovered in the vicinity of 2.4 grams per total gallon. The Department of
Health, Education and Welfare (HEW), which was home to the Surgeon General
starting with the Kennedy Administration, had authority over lead emissions
under the Clean Air Act of 1963. The criteria mandated by this statute were
still in the draft stage when the Act was reauthorized in 1970 and a new agency called EPA came into existence.
By then, the adverse effects of America's decades-old
addiction to fossil fuel in general and leaded fuel in particular were becoming obvious to all. In 1971, the EPA's first Administrator, William D. Ruckelshaus, declared that "an extensive body of information exists which indicates that the addition of alkyl lead to gasoline...results in lead particles that pose a threat to public health."
It should be emphasized, however, that scientific evidence capable of documenting this conclusion did not exist in previous decades. Only very recently have scientists been able to prove that low-level lead exposure resulting from automobile emissions is harmful to human health in general, but especially to the health of children and <A href="http://www.medicinenet.com/script/main/art.asp?articlekey=10695">pregnant</a> women.
EPA took an emphatic stand on the issue in its final
health document on the subject, "EPA's Position on the Health Implications of
Airborne Lead," which was released on November 28, 1973. This study confirmed
what preliminary studies had already suggested: namely, that lead from
automobile exhaust was posing a direct threat to public health. Under the Clean
Air Amendments of 1970, that conclusion left EPA with no option but to control
the use of lead as a fuel additive known to "endanger the public health or welfare."
The very next month, in December 1973, EPA issued regulations calling for a gradual reduction in the lead content of the total gasoline pool, which includes all grades of gasoline. The restrictions were scheduled to be implemented starting on January 1, 1975, and to extend over a five-year period. The average lead content of the total gasoline pool of each refinery was to be reduced from the level of approximately 2.0 grams per total gallon that prevailed in 1973 to a maximum of 0.5 grams per total gallon after January 1, 1979. Litigation was to postpone implementation of this phasedown for two years.
Starting with the 1975 model year, US automakers responded to EPA's lead phasedown timetable by equipping new cars with pollution-reducing catalytic converters designed to run only on unleaded fuel. Fittingly, a key component of these catalysts that were to be the undoing of lead was that noblest of noble metals, platinum.
Although over 40% of all pump sales are still leaded as
of today, the market share of leaded vehicle is steadily diminishing. And with
it, so is the noxious cloud of lead-polluted air we have grown accustomed to
breathing. EPA estimates that ambient lead levels dropped 64 percent between 1975 and 1982.
In 1982, with the introduction of unleaded gasoline well underway, EPA developed a new standard intended to apply strictly to leaded gasoline.
On the basis of all that is known about the history of lead and its adverse effects on human health, it is impossible not to welcome EPA's lead phasedown initiative as well as the agency's decision to consider banning lead altogether from US gasoline.

Lead poisoning

Lead poisoning: An environmental hazard capable of causing brain
damage. In the US lead poisoning is formally defined as having at least 10
micrograms of lead per deciliter of blood. (The average level of lead, for
people ages 1 to 70, is 2.3 micrograms.)
The lead may come, for example, from lead-containing paint, leaded gasoline,
etc. Lead was used in household paint until 1978 and was also found in leaded
gasoline, some types of batteries, water pipes, and pottery glazes. Lead paint
and pipes are still found in many older homes and lead is sometimes also found
in water, food, household dust and soil. Lead can be a workplace hazard for
people in certain occupations.
Diagnosis is by blood test: Blood lead levels of 10 ug/dL or greater are
dangerous to children, even if the person has no apparent symptoms.
A diet that is high in iron and calcium can help protect people against
absorbing lead. Treatment of lead poisoning is by chelation therapy, usually in
the hospital. Treatment cannot repair damage to the brain done by lead
poisoning, but may prevent further damage.
If you think your home has high levels of lead:

<li>Have your children tested for lead, even if they seem healthy.</li>
<li>Wash children's hands, bottles, pacifiers, and toys often.</li>
<li>Make sure children eat healthy, low-fat foods.</li>
<li>Get your home checked for lead hazards.</li>
<li>Regularly clean floors, window sills, and other surfaces.</li>
<li>Wipe soil off shoes before entering house.</li>
<li>Talk to your landlord about fixing surfaces with peeling or chipping
paint.</li>
<li>Take precautions to avoid exposure to lead dust when remodeling or
renovating.</li>
<li>Don't use a belt-sander, propane torch, dry scraper, or dry sandpaper on
painted surfaces that may contain lead.</li>
<li>Don't try to remove lead-based paint yourself.</li>

damaging effects of lead.</li>

<li>Many homes built before 1978 have lead-based paint. The federal government
banned lead-based paint from housing in 1978. Some states stopped its use
even earlier.</li>
<li>Lead-based paint that is in good condition is usually not a hazard.</li>
<li>Peeling, chipping, chalking, or cracking lead-based paint is a hazard and
needs immediate attention.</li>
<li>Lead dust can form when lead-based paint is dry scraped, dry sanded or
heated. Dust also forms when painted surfaces bump or rub together. Lead
chips and dust can get on surfaces and objects that people touch. Settled
dust can reenter the air when people vacuum, sweep, or walk through it.
Contaminated bare soil can also increase dust lead levels.</li>

Historical Note: Dr. Julian Chisolm (1921-2001) is credited with

treatment called chelation in 1968. It remains the primary way to remove lead
from the blood in severe lead poisoning. Chelation involves injecting a chemical
into the blood that bonds with the lead, forming a compound that can be excreted
in urine. Chisolm also devised a finger-stick test that made it possible to
diagnose the early stages of lead poisoning in children quickly and
economically.
Current Situation: While severe lead poisoning has now largely
disappeared in the US, low-level poisoning remains a problem, especially in
inner-city housing built before 1960. A study done by the CDC (Centers for
Disease Control and Prevention) in 1999 found that nearly 900,000 American
preschoolers had lead levels that could hurt their health or their ability to
